When the full range of programs are running, the Community Wellbeing service offers a range of community transport and social connection opportunities for residents over the age of 65 or under 65 living with a disability.

Community transport responds to the needs of those with limited access to conventional means of public and private transport. Our personalised, friendly and reliable community transport options can help you meet important commitments, attend appointments, do your shopping or visit the library. Maintaining social connections and taking part in meaningful activities with others is essential to enjoying life. Our team also facilitates the Cumberland Crew social group aimed at providing fun and engaging activities for residents aged under 65 with a disability.


How do you apply?

My Aged Care – 1800 200 422

If you are seeking a service for the first time, you can apply through the Commonwealth Government gateway, ‘My Aged Care’. Eligibility is determined on the basis of age (65 years or over, or 50 years if of Aboriginal or Torres Strait Islander descent) and your need for assistance with living independently at home and getting out and about in the community. My Aged Care will register you over the phone and in most cases arrange a more detailed face to face assessment. When registering you will be given the opportunity to choose who provides your services.

If you would like assistance with contacting My Aged Care, please contact us on 8372 8888. If you are under the age of 65 (or 50 if of Aboriginal or Torres Strait Islander descent) you can contact us on 8372 8888. We will determine your eligibility and conduct an assessment.
